An RMA record should let another reviewer understand the original symptom without reconstructing the entire incident. Distinguish a module fault from host configuration, cabling and peer-end issues.
Identification
Retain order, model, serial number, label photographs and installation date.
Measurements
Attach timestamped link state, counters, telemetry and settings; describe the tool and test conditions.
Handling
Record controlled swaps, cleaning and packaging. Preserve the failed state when further testing could remove useful evidence.
